要約(日本語): 本論文では，スマートフォンやインターネットなどの依存症に対して，高校生から大学生を主たる対象として，我々が展開している活動を報告する．これらの依存症は若い世代において，深刻な問題になりつつある．本活動では，これらの若い世代の実態を知るために，独自のアンケートを開発し実施した．その結果にもとづき，高校生と大学生の公開討論会を実施し対策について議論した．本論文では，アンケート設計の詳細と結果の分析結果について述べる．なお，本活動は神奈川県の大学発政策提案制度に採択されて実施している．

要約(英語): We in this paper report the activity whose main purpose is a treatment for smartphone and the Internet addiction. This type of addiction is now becoming a social problem, and the situation is serious especially in younger generations such as high school and university students. We developed and implemented an original questionnaire. We describe details of the questionnaire design, and provide analysis results. This activity is selected Kanagawa prefecture as policy proposals from universities.
